Everything You Need to Know to Make a Quilt Square

Please print and complete a Threads of Life Quilt Square Form. The following are the specifications for your quilt square:

• Quilt squares must be an 8" square (maximum), allowing for a ½" border.

 • The pattern or design should stay within 7".

• Any color of fabric may be used - bright colors tend to stand out better.

• You may use any medium you choose to design the square, such as fabric, crayons, paint, markers, thread, etc.

• You may include the recipient’s name, type and date of transplant in your design.

• Squares must be dry cleanable.

• Please make sure your name, address, phone number and e-mail are included with your square so we may contact you when the quilt is completed.
