Joining the Ohio Donor Registry

Not certain that you are a registered organ, eye or tissue donor? 

Look for the heart symbol on the bottom right corner of your Ohio driver license or state ID.  You can also verify your status online using the link below.

To become a registered organ, tissue and eye donor now, click on the first option.  Not sure you are ready or you have questions?  Click on option two or three.  Prefer to print a form and mail it in? Option four is for you!  For those that live out of the state of Ohio, Donate Life America will help identify the requirements in your area.  Choose option five.
